We want an independent body to establish facts, identify failings and refer misconduct to relevant authorities. When failures span several organisations, no independent body examines the whole process. This can leave witness contamination, Section 28 breaches, false evidence, missing records and safeguarding failures across the criminal legal process without proper investigation. The proposed body could establish facts, identify systemic failings, refer suspected misconduct and recommend improvements.
