Review Government Departments’ handling of prevention of future deaths reports — 22 signatures. Status: open.
We call for a review into how Government departments handle prevention of future deaths reports. We are concerned that there have been multiple instances where such reports have not been acted on.
In one such instance, multiple prevention of future deaths reports were submitted to Government Departments to warn them about a harmful online suicide forum which promoted the sale of poison. We believe the Government did not do enough to act on this. We therefore call for a review to ensure prevention of future deaths reports are met with the immediate and serious response they require, to prevent future harm.
