Require the HSE to publish national guidance on standardised near miss reporting in complex socio-technical systems. The guidance should support consistent reporting, systems thinking and organisational learning from both risk identification and successful risk mitigation.
Near-miss reporting is widely encouraged but there is no national guidance on how organisations should systematically analyse and learn from these events, particularly in complex socio-technical systems. I ask Parliament to support the HSE in developing guidance to standardise near-miss reporting and promote systems thinking, helping organisations learn from both risks that are identified and the actions that successfully prevent harm, improving safety and organisational learning.
