Too many vulnerable patients are being discharged from hospital without adequate assessments to ensure their safety, wellbeing, support arrangements, and ability to cope once they return home or move to another care setting.
We believe hospitals should be legally required to complete robust safeguarding and discharge assessments before any vulnerable patient is discharged. These assessments should involve relevant family members, carers, social workers, community healthcare professionals, and safeguarding teams wherever appropriate, ensuring that decisions are made collaboratively and in the patient's best interests.
